Dear Patient,
I would like to thank you for your trust and loyalty these past 33 years. It has been an honor and a privilege to care for you and your family here at The Eye Clinic Surgicenter and to serve our community. I will be retiring at the end of December, but your health and wellness will continue to be a priority for us.
It is important that you continue with appropriate medical care; therefore, you should establish contact with another physician as soon as possible.
In order to help you find another doctor, the following information is provided:
Option I: You may wish to contact an Ophthalmologist at Billings Clinic. They have a lot of doctors available including Dr Frewin, Weaver, Cornetet. They can be reached at 406-238-2500 ask for the Ophthalmology department
Option II: Establish with another Ophthalmologist in the area, please visit the Montana Academy of Ophthalmology website to find one that fits your needs/location at www.montanaeyemds.org.
Option III: If you are in a managed care situation, you may need to contact your healthcare insurer for a referral within the network.
RECORDS:
We have planned for Billings Clinic Hospital to be your records custodian. After January 1, 2026, your ophthalmology records will be available at Billings Clinic Hospital. If you schedule your next eye appointment with their ophthalmology department, they should easily access your records. Alternatively, you can request a copy of your ophthalmology records from them if desired, any time after the new year.
If you desire your records, be sent to you or a new provider there is a charge for this service (see below). We have enclosed a HIPAA-compliant authorization form that is necessary to release a copy of your medical record to you or your new physician. Please complete the form and return it as soon as possible. On receipt of this signed form, my office will forward a copy of your medical records to the physician you designate. Please rest assured that I will remain available during the next 30 days to consult with your new provider to promote a smooth transition and continuity of care.

If you choose to pick up a copy of your medical records, give us a 30-day notice and they will be available for pickup until December 30th at our office. You will need the HIPAA release form filled out. Please be aware that the charge to send records to your new physician or to pick up a personal copy is $21.50.
If you have not selected a new physician by the time I retire and have an emergency, you can obtain emergency care by calling the Billings Clinic Hospital Emergency Department or St Vincent Hospital Emergency Department.
